Amy Jensen PMHNP-BC
I take the whole person in to account when dealing with mental health. I have over 13 years experience as a Psychiatric Nurse Practitioner and a total of 22 years in the mental health field. I am Board Certified by ANCC.
I specialize in adults with chronic and acute mental health concerns such as depression, bipolar disorder, personality disorders, schizophrenia, schizoaffective, anxiety and I have a subspecialty in adults with developmental disabilities and autism. I provide a thorough and ongoing evaluation of current symptoms and stressors and work with each person towards their individualized goal of treatment, whether that is via medication management or therapy or a combination of both.
Currently services are offered through telehealth only.
Superbilling is provided per request for reimbursement from your insurance (please contact your insurance directly for reimbursement request) If your plan offers "out-of-network" coverage, I do not bill the insurance for you. I provide you with a “superbill” payment invoice, upon payment. You are then able to submit this receipt to your insurance company for direct reimbursement.

Practice Style/Managing Expectations
Please note that pharmacotherapy may or may not be clinically appropriate or recommended for your issues and condition(s). I do not guarantee that I will continue or refill any medications that is clinically unsafe or harmful for your condition, especially on a first visit and/or if there has been acute changes. If you have more than one prescriber at the same time, or have recently had several different doctors we will need to discuss to better understand why. NOTE: I do not refill or manage stimulants, non-psychiatric medications such as opioids or other types of pain management and I am very cautious about the use of controlled medications.
Your safety and overall health is my priority. Clients who are in treatment with my practice are receiving care voluntarily, agree to be adherent to treatment plans, and should be stable enough to receive care in an outpatient telehealth setting. If you are in need of acute treatment services or other treatments that are outside the scope of my solo practice, license, and clinical expertise - I will recommend that you get the necessary care to better help you.
If your symptoms are severe or life-threatening, please get help now. If your condition becomes unstable, medically complex beyond my scope of practice, or could benefit from a specialist setting or meeting with a physician, I will be transparent and recommend transferring care so you could get the best care possible.
YOU and your overall health are important to me!
Late Cancellation/No Show Fee is $100-175. If you don't show or call, please be aware that you will be charged a standard fee for the missed appointment. You must cancel/reschedule with at least 24 hours notice, as I am able to contact others on my waiting list to fill the spot.
If there is an urgent situation or unexpected medical or personal emergency that you needed to cancel your appointment - please don't hesitate to contact me by phone or by email, so we can work together to find a workable solution.
*Refill request outside of appointments is $50. Please pay close attention to when you are running low or out of medication and do your best to schedule an appointment BEFORE you run out of medication. Please make sure to allow at least 5-7 days at minimum before you run out of medications. I do not routinely refill your medications outside of appointments, especially, if they are controlled substances that require in-person monitoring and check-ins. If there is a pattern of refill requests without appointments, my private practice may not be the best fit for your treatment, and I would be happy to provide you with referrals to colleagues working in larger group practices or local clinics, who tend to have additional on-call services and the benefit of multiple providers.
*Forms and paperwork, phone consults - outside of appointments are billed based on my standard hourly rate of $300. This is reserved for extraordinary clinical circumstances such as complex case coordination, comprehensive clinical and legal forms, and/or extensive phone consultations outside of appointments, exceeding 10 minutes per week. It is best to address forms, paperwork and consults together during a scheduled appointment.
As a part-time PMHNP-BC in solo private practice, my time, resources, and scope of practice are more limited than treatment services at a larger clinic or group practice. There is no charge for brief and routine telephone calls for administrative tasks such as scheduling appointments or for billing questions.
Payments
Payment is due at the time of service. If there is an unpaid balance, I will ask you to provide payment during your visit to cover this. This includes any unpaid visit balances. I am set up with Venmo for payments currently.
If you are more than 90 days past due with a visit balance, I will do my best to reach you to address payment before any future appointments are scheduled. If you do not contact me after several attempts, your balance may be 1) turned over to a collections service and/or 2) need to terminate our treatment relationship due to non-payment. Let's please stay in touch to avoid this!
We can review treatment policies, fees/billing and any other "housekeeping" questions during your intake.
